Summary: Monkeypox virus (MPXV) has been circulating in West and Central Africa. Recent outbreaks in nonendemic countries were analyzed, revealing that the MPXV-2022 strains belong to the same lineage as the 2018 strain with several new mutations. Some specific proteins were found to be more prone to mutation in the MPXV genome. This study provides insights into the ongoing MPXV outbreak and identifies novel mutations for further research.

Epidemiological, clinical and virological characteristics of four cases of monkeypox support transmission through sexual contact, Italy, May 2022

Summary: Since May 2022, there has been an ongoing monkeypox outbreak in non-endemic countries. Four cases in young adult men in Italy, who engaged in condomless sexual intercourse, were reported. The patients are in good clinical condition and do not require specific antiviral drugs. Monkeypox viral DNA was detected in seminal fluid samples. The possibility of sexual transmission of monkeypox virus needs to be investigated.

Community transmission of monkeypox in the United Kingdom, April to May 2022

Summary: Between 7 and 25 May, 86 cases of monkeypox have been confirmed in the UK, with only one case known to have traveled to a monkeypox virus endemic country. The majority of the cases were males, among which 66 reported being gay, bisexual, or men who have sex with men. This is the first sustained human-to-human transmission of monkeypox in the UK, occurring through close contacts, including sexual networks. Ongoing efforts are being made to improve case identification and prevent further transmission.

Clinical features and management of human monkeypox: a retrospective observational study in the UK

Summary: This study retrospectively observed the clinical course of monkeypox in seven patients in the UK from 2018 to 2021. The study found that prolonged PCR positivity led to isolation of five patients for more than 3 weeks. Three patients treated with brincidofovir developed elevated liver enzymes, leading to termination of the therapy. One patient treated with tecovirimat experienced no adverse effects and had a shorter duration of viral shedding and illness compared to the other six patients.

No secondary cases were found among close contacts.

Monkeypox outbreak in Madrid (Spain): Clinical and virological aspects

Summary: This observational study focused on monkeypox cases in Madrid, Spain, and investigated the clinical and virological aspects of the disease. The findings revealed that the virus circulating in Spain belongs to the western African clade, mainly affecting cisgender men who have sex with men (MSM). The first symptom of the disease was skin lesions, and the disease followed a self-limited course.

Phylogenomic characterization and signs of microevolution in the 2022 multi-country outbreak of monkeypox virus

Summary: In this study, shotgun metagenomics were used to rapidly reconstruct and characterize the genome sequences of the largest monkeypox virus outbreak in non-endemic countries so far. The findings indicate that the outbreak most likely has a single origin and that the virus belongs to clade 3. Furthermore, the study suggests the involvement of host APOBEC3 in viral evolution and reveals signs of ongoing microevolution in the virus.

However, a majority of them were in favor of using variola vaccine to prevent MPX, and previous vaccination against seasonal influenza and willingness to receive variola vaccine were identified as the main factors influencing their attitude.
