Summary: Evaluation of surgery for congenital craniovertebral junction anomalies with atlantoaxial dislocation/basilar invagination showed that preoperative poor Nurick grade is a risk factor for poor outcome. For patients with irreducible AAD/BI, transoral odontoidectomy with posterior fusion and only posterior fusion had no significant differences in perioperative complications, outcomes, and fusion.
