Cost-effectiveness analysis of endovascular treatment with or without intravenous thrombolysis in acute ischemic stroke

Adnan Qureshi et al.

Summary: This study evaluated the cost-effectiveness of endovascular treatment (EVT) alone compared with EVT following intravenous (IV) recombinant tissue plasminogen activator (r-tPA) in patients with acute ischemic stroke (AIS). The results showed that EVT alone was more cost-effective than EVT and IV r-tPA combined treatment.

Summary: This study compares the effectiveness of endovascular thrombectomy (EVT) with intravenous thrombolysis (IVT) or IVT alone in minor stroke patients. The results suggest similar functional outcomes between IVT alone and EVT with or without IVT.

Effect of Intra-arterial Alteplase vs Placebo Following Successful Thrombectomy on Functional Outcomes in Patients With Large Vessel Occlusion Acute Ischemic Stroke The CHOICE Randomized Clinical Trial

Arturo Renu et al.

Summary: This study investigated whether treatment with adjunct intra-arterial alteplase after thrombectomy improves outcomes following reperfusion. The results showed that compared with placebo, the use of adjunct intra-arterial alteplase resulted in a greater likelihood of excellent neurological outcome at 90 days.

Direct endovascular treatment versus bridging therapy in patients with acute ischemic stroke eligible for intravenous thrombolysis: systematic review and meta-analysis

Jian Zhang et al.

Summary: This review and meta-analysis compared the efficacy and safety of direct EVT and bridging therapy for IVT-eligible patients with AIS-LVO, finding no significant differences in outcomes such as mRS scores, mortality, sICH, and successful recanalization between the two treatment strategies. However, patients treated with direct EVT had a lower risk of any type of ICH and clot migration compared to those receiving bridging therapy.

Stroke prevention and control system in China: CSPPC-Stroke Program

Bao-Hua Chao et al.

Summary: The China Stroke Prevention Project Committee has been actively promoting stroke prevention and control in China. With over 8.3 million people screened and followed up from 2012 to 2018, the committee has helped establish 380 stroke centers in mainland China and improved stroke care quality through monitoring and reporting. Further nationwide promotion of the program is needed to continue the progress.

Wenjie Zi et al.

Summary: This study showed that endovascular treatment alone was noninferior to combined intravenous alteplase and endovascular treatment in achieving functional independence at 90 days among patients with ischemic stroke due to proximal anterior circulation occlusion within 4.5 hours from onset, with no significant differences in safety outcomes.

Kentaro Suzuki et al.

Summary: In patients with acute large vessel occlusion stroke, mechanical thrombectomy alone did not show noninferiority compared to combined intravenous thrombolysis plus mechanical thrombectomy in terms of favorable functional outcomes. However, there was also no evidence of inferiority due to wide confidence intervals.

Summary: This study aimed to compare the real-world clinical outcomes of patients with acute ischemic stroke treated with direct ET and BT in China. The results showed that patients in the BT group had shorter treatment times and higher rates of rapid neurological improvement, while also experiencing a higher risk of intracranial hemorrhage compared to the direct ET group.
