Long-term structural and symptomatic effects of intra-articular sprifermin in patients with knee osteoarthritis: 5-year results from the FORWARD study

Summary: After a 5-year follow-up, sprifermin showed a significant dose-response effect on total femorotibial joint cartilage thickness, with better efficacy seen in the sprifermin 100 μg group compared to placebo. Improvement in WOMAC pain scores of approximately 50% from baseline was observed in all treatment groups, with no knee replacements in the 100 μg group. Adverse events were reported in the majority of patients in both sprifermin and placebo groups, but they were mostly mild or moderate and not related to treatment.

Summary: Through a large-scale evaluation of plasma proteins, a novel potential biomarker CRTAC1 associated with both diagnosis and progression to joint replacement in OA patients was identified. Patients with OA in the highest quintile of risk for joint replacement based on age, sex, BMI, and plasma CRTAC1 level were significantly more likely to undergo knee or hip replacement within 5 years compared to those in the lowest quintile.

By using MRI-based morphological phenotypes, it is possible to predict future OA incidence and improve treatment efficacy.
