Underestimation of Aortic Stenosis Severity by Doppler Mean Gradient during Atrial Fibrillation: Insights from Aortic Valve Weight

Doppler Mean Gradient Is Discordant to Aortic Valve Calcium Scores in Patients with Atrial Fibrillation Undergoing Transcatheter Aortic Valve Replacement

Summary: Patients with atrial fibrillation (AF) may have underestimated aortic stenosis (AS) severity at the time of referral for transcatheter aortic valve replacement (TAVR). This study compared flow-independent computed tomographic aortic valve calcium scores (AVCS) to mean gradient (MG) in AF versus sinus rhythm (SR) in patients undergoing TAVR. The results showed that AVCS were higher than expected by MG in AF, particularly in men with high-gradient AS (HGAS). This suggests late diagnosis of severe AS in men with AF and HGAS. Further studies are needed to examine the extent of AS severity underestimation by echocardiography in AF.

Summary: The study found that severe valve stenosis was common in atrial fibrillation patients with low-gradient aortic valve stenosis. In these patients, computed tomography aortic valve calcium scores were not significantly different compared to patients with sinus rhythm, but mortality was higher in patients with atrial fibrillation and low-gradient aortic stenosis when high Doppler signals were present.
