Risk predictors in a Spanish cohort with cardiac laminopathies. The REDLAMINA registry

Summary: This study aimed to describe the clinical characteristics of carriers of LMNA genetic variants in a Spanish cardiac-laminopathies cohort and assess risk criteria. The results showed that LVEF <45% and NSVT were associated with major arrhythmic events, while sex or type of genetic variant were not. Female carriers of missense variants with NSVT or LVEF <45% should not be considered a low-risk group. Individualized risk stratification is important for carriers of LMNA missense variants, as prognosis varies among individuals.

Association of Genetic Variants With Outcomes in Patients With Nonischemic Dilated Cardiomyopathy

Summary: This study found that DCM patients with pathogenic or likely pathogenic variants had worse prognosis than genotype-negative individuals. Clinical outcomes varied depending on the underlying affected gene, and individuals with baseline left ventricular ejection fraction ≤35% who had pathogenic variants were more likely to experience major adverse cardiovascular events, ESHF, and MVA compared to genotype-negative patients.

Improved Risk Stratification for Ventricular Arrhythmias and Sudden Death in Patients With Nonischemic Dilated Cardiomyopathy

Summary: In this study, delayed gadolinium enhancement (LGE) was found to be a significant, consistent, and strong predictor of ventricular arrhythmias (VA) or sudden death in patients with dilated cardiomyopathy (DCM). A new clinical algorithm integrating LGE and left ventricular ejection fraction (LVEF) significantly improved risk stratification, with relevant implications for implantable cardioverter-defibrillator allocation.

Clinical Risk Prediction in Patients With Left Ventricular Myocardial Noncompaction

Summary: This study confirmed the association of LVNC with an increased risk of heart failure and ventricular arrhythmias, with left ventricular ejection fraction being the variable most strongly associated with MACE. Late gadolinium enhancement (LGE) confers additional risk in patients without severe systolic dysfunction. A risk prediction model was developed and validated in order to guide management.
