Stroke Mimics in the Acute Setting: Role of Multimodal CT Protocol

E. Prodi et al.

Summary: Multimodal CT has low sensitivity but high specificity in the diagnosis of stroke mimics in the acute setting. The high specificity of multimodal CT allows ruling out stroke and avoiding unnecessary revascularization treatment in patients with a diagnosis of a stroke mimic.

Apparent posterior cerebral artery territory perfusion asymmetry on arterial spin labeling MRI is a common non-pathologic finding in patients with a unilateral fetal posterior cerebral artery

Abraham Noorbakhsh et al.

Summary: This study aimed to determine the frequency of apparent PCA territory asymmetry in patients with unilateral fetal PCA on ASL imaging. The results showed that a subset of patients may exhibit apparent hypoperfusion in the PCA territory. Radiologists should be aware of this phenomenon to avoid misdiagnosis when interpreting imaging findings.

Summary: Differentiating stroke from stroke mimics, such as Posterior Reversible Encephalopathy Syndrome (PRES), is a diagnostic challenge. MR perfusion (MRP) imaging modalities, particularly Arterial Spin Labeling (ASL), could provide added value in detecting PRES.

Arterial Spin Labeling technique and clinical applications of the intracranial compartment in stroke and stroke mimics - A case-based review

Laleh Daftari Besheli et al.

Summary: Magnetic resonance imaging perfusion (MRP) techniques, particularly arterial spin labeling (ASL), can improve the selection and evaluation of acute ischemic stroke patients for treatment. ASL, a noncontrast MRP technique, allows assessment of cerebral blood flow without the use of intravenous contrast. It is particularly useful in stroke imaging for evaluating core infarct and penumbra, localizing vascular occlusion, and assessing collateral status.

Predictors of perfusion computed tomography alterations in stroke mimics attended as stroke code

Alicia Gonzalez-Martinez et al.

Summary: This study evaluated clinical characteristics and PCT alterations in patients attended as stroke code but diagnosed with stroke mimics. PCT abnormalities were found in almost a third of stroke mimics, more prevalent in older women with aphasia. The main patterns of PCT alterations included changes in time to peak map, unilateral hemispheric distribution, or non-vascular territory.

Automated Brain Perfusion Imaging in Acute Ischemic Stroke Interpretation Pearls and Pitfalls

Manal Nicolas-Jilwan et al.

Summary: Recent advancements in computed tomography technology have reinforced the importance of perfusion computed tomography imaging in evaluating patients with acute ischemic stroke, especially in selecting patients for endovascular thrombectomy. Studies have shown the benefits of perfusion imaging in diagnosing patients with anterior circulation large vessel occlusion within 24 hours. This compelling data has led to an increased introduction of automated perfusion computed tomography imaging in routine evaluation of stroke patients.

Non-Contrast-Enhancing Glioma Mimicking Acute Ischemic Stroke Within Three Hours of Onset

Summary: Intravenous thrombolytic therapy is recommended within 3 hours of onset of acute ischemic stroke, but stroke mimics may pose challenges. Some brain tumors are rarely seen on a brain CT scan, and a mismatch between neurological examination and brain perfusion imaging may suggest a stroke mimic.
