Hydroxyethyl Starch for Fluid Management in Patients Undergoing Major Abdominal Surgery: A Systematic Review With Meta-analysis and Trial Sequential Analysis

Joris Pensier et al.

Summary: This study found that HES intravascular volume replacement is not associated with an increased risk of 30-day AKI compared to crystalloid intravascular volume replacement in major abdominal surgery, but it is associated with higher rates of blood transfusion and similar rates of postoperative intra-abdominal complications.

Balanced Multielectrolyte Solution versus Saline in Critically Ill Adults

Simon Finfer et al.

Summary: There is no evidence that the use of balanced multielectrolyte solution (BMES) reduces the risk of death or acute kidney injury in critically ill patients compared to saline, according to a double-blind, randomized, controlled trial.

Guidelines for the choice of intravenous fluids for vascular filling in critically ill patients, 2021

Olivier Joannes-Boyau et al.

Summary: This article provides recommendations for the appropriate choice of fluid therapy for resuscitation of critically ill patients. A consensus committee of experts was convened to develop these recommendations, and the Grading of Recommendations Assessment, Development and Evaluation (GRADE) system was used to assess the quality of evidence. The guidelines cover four fields and include nine statements, with strong agreement reached for all recommendations. The recommendations aim to optimize fluid therapy choices for critically ill patients.

Restriction of Intravenous Fluid in ICU Patients with Septic Shock

T. S. Meyhoff et al.

Summary: The study found that restricting intravenous fluid therapy in adult patients with septic shock in the ICU did not lead to fewer deaths at 90 days compared to standard intravenous fluid therapy. Other survival indicators during ICU stay and at 90 days were similar in both groups.

Effect of Change in Body Weight on Clinical Outcomes in Critically Ill Patients

Rajesh K. Mishra et al.

Summary: This study investigated the association between body weight changes and fluid balance in ICU patients. It found that weight gain on the third and fourth days of ICU admission was associated with increased ICU mortality, and that body weight changes showed a good correlation with cumulative fluid balance.

Effect of 0.9% NaCl compared to plasma-lyte on biomarkers of kidney injury, sodium excretion and tubular transport proteins in patients undergoing primary uncemented hip replacement - a randomized trial

A. M. Ostergaard et al.

Summary: The study showed that despite no significant differences in kidney injury markers and creatinine, urinary NGAL and urinary KIM-1 significantly increased in both groups after surgery, indicating potential subclinical kidney injury. Additionally, the IS group exhibited delayed sodium excretion compared to the PL group, suggesting a differential effect of IS on renal sodium excretion.

Fluid-induced harm in the hospital: look beyond volume and start considering sodium. From physiology towards recommendations for daily practice in hospitalized adults

Niels Van Regenmortel et al.

Summary: Iatrogenic fluid overload, primarily caused by sodium administration, can have detrimental effects on hospitalized patients. Strategies such as using hypotonic maintenance fluids and minimizing sodium caused by fluid creep can help mitigate the risks associated with excessive sodium intake.

Perioperative hemodynamic optimization: from guidelines to implementation-an experts' opinion paper

Jean-Luc Fellahi et al.

Summary: Despite limited implementation of guidelines on hemodynamic optimization, experts propose a method based on six questions to accelerate this implementation. They recommend maintaining mean arterial pressure above 65mmHg during surgery to reduce the risk of postoperative organ dysfunction. The use of dynamic indices for fluid therapy, along with individualized monitoring, is suggested to optimize treatment.
