Summary: Non-steroidal mineralocorticoid receptor antagonists (MRAs) show promising therapeutic effects in the treatment of diabetic kidney disease (DKD), including reducing albuminuria, slowing CKD progression, and lowering the risk of adverse cardiovascular outcomes. Compared with steroidal MRAs, non-steroidal MRAs carry a lower risk, providing potential for their use in the treatment of DKD.

Interrelationship between renin-angiotensin-aldosterone system and oxidative stress in chronic heart failure patients with or without renal impairment

Clinical studies have shown the benefits of MR antagonists for DKD, but concerns remain about their selectivity and hyperkalemia risks. New nonsteroidal MRAs, such as finerenone, have been developed with high selectivity and potent anti-inflammatory and anti-fibrotic properties, and a lower risk of hyperkalemia.

The study found that aldosterone reduces vascular Nrf2 transcriptional activity by Keap1-dependent mechanisms, leading to mineralocorticoid-induced vascular dysfunction. Activation of Nrf2 with SFN can reverse the vascular dysfunction and ROS generation induced by aldosterone.
