A novel classification in predicting prognosis and guiding postoperative management after RO liver resection for patients with hepatocellular carcinoma and microvascular invasion

Kang Wang et al.

Summary: This study aimed to establish a novel classification of microvascular invasion (MVI) for predicting prognosis in hepatocellular carcinoma (HCC) patients after RO liver resection (LR). The classification, developed using multivariate cox regression analysis, included factors such as alpha-fetoprotein level, liver cirrhosis, tumor characteristics, and MVI features. The classification system categorized patients into three distinct survival groups and showed higher predictive accuracy compared to other commonly used classifications.

Adjuvant stereotactic body radiotherapy after marginal resection for hepatocellular carcinoma with microvascular invasion: A randomised controlled trial

Changying Shi et al.

Summary: This study aimed to investigate the efficacy of SBRT targeting suboptimal resection margin as adjuvant therapy in MVI-positive HCC. The results showed that SBRT group had significantly better DFS and OS compared to the surgery alone group.

Image-matching digital macro-slide-a novel pathological examination method for microvascular invasion detection in hepatocellular carcinoma

Hong-Ming Yu et al.

Summary: This study demonstrates that the use of IDS, a novel pathological examination method combining macro-slide and whole-mount slide imaging, can enhance the detection rate of MVI in hepatocellular carcinoma (HCC) and improve the prediction of HCC prognosis. The study also identifies alpha-fetoprotein as a suitable and robust biomarker to recognize MVI false-negative patients in conventional pathological protocols.

Actual long-term survival in hepatocellular carcinoma patients with microvascular invasion: a multicenter study from China

Zhen-Hua Chen et al.

Summary: One-third of HCC patients with MVI achieved the long-term survival milestone of 5 years after resection. Anatomical hepatectomy, controlling intraoperative blood loss, a wide resection margin, and postoperative adjuvant TACE should be considered to improve long-term survival outcomes.

Prognostic Value of Microvascular Invasion in Eight Existing Staging Systems for Hepatocellular Carcinoma: A Bi-Centeric Retrospective Cohort Study

Yan-Jun Xiang et al.

Summary: Microvascular invasion (MVI) significantly impacts the prognosis of patients with hepatocellular carcinoma (HCC) after R0 liver resection. Among the eight commonly used staging systems, the Hong Kong Liver Cancer (HKLC) staging system showed the highest accuracy in predicting the prognosis of MVI patients. Incorporating MVI into current HCC staging systems may be necessary based on the findings.
