Prediabetes and risk of mortality, diabetes-related complications and comorbidities: umbrella review of meta-analyses of prospective studies

Sabrina Schlesinger et al.

Summary: The term prediabetes refers to individuals with impaired glucose metabolism who have not yet reached the glucose or HbA(1c) levels required for a diabetes diagnosis. A systematic review of meta-analyses found that prediabetes is associated with an increased risk of all-cause mortality and various diabetes-related complications, including cardiovascular outcomes, cancer, and dementia. More high-quality studies on HbA(1c)-defined prediabetes and other relevant health outcomes are needed to further support these findings.

Summary: This study examined the characteristics and outcomes of patients with diabetes or pre-diabetes in HFpEF and found that pre-diabetes is common in these patients and is associated with worse clinical status and higher risk of heart failure hospitalizations.

Prediabetes and insulin resistance in a population of patients with heart failure and reduced or preserved ejection fraction but without diabetes, overweight or hypertension

Tran Kim Son et al.

Summary: This study investigated the prevalence of prediabetes and insulin resistance (IR) in Vietnamese patients with heart failure (HF) and no overweight, diabetes, or hypertension. The study found that HF patients had a higher prevalence of prediabetes and IR compared to healthy controls. Furthermore, prediabetes and IR were associated with a more severe HF. These findings suggest that HF may play a role in the development of IR and emphasize the importance of intervention strategies for HF patients with prediabetes.

Effects of Long-term Metformin and Lifestyle Interventions on Cardiovascular Events in the Diabetes Prevention Program and Its Outcome Study

Ronald B. Goldberg et al.

Summary: The study found that neither lifestyle intervention nor metformin reduced the incidence of major cardiovascular events over a 21-year period, despite their long-term diabetes prevention effects. It is possible that the provision of group lifestyle intervention to all participants, extensive use of statin and antihypertensive agents outside the study, and a decrease in the use of study metformin over time may have diluted the effects of these interventions.

Summary: The remission of type 2 diabetes is crucial for reducing the risks of complications, which can be achieved through early detection and significant weight loss during a 6-year therapeutic window while the beta-cells are still functional.

Type 2 Diabetes and Cancer: An Umbrella Review of Observational and Mendelian Randomization Studies

Jonathan Pearson-Stuttard et al.

Summary: The study summarized evidence from observational and Mendelian randomization studies, finding a clear association between type 2 diabetes and colorectal, hepatocellular, gallbladder, breast, endometrial, and pancreatic cancers. Potential causal associations were identified for genetically predicted T2DM and fasting insulin concentrations with endometrial, pancreas, kidney, breast, lung, and cervical cancer risk.

Summary: The 1-h PG of >= 11.6 mmol/L during OGTT has good sensitivity and specificity for detecting type 2 diabetes. Prescreening with a diabetes-specific risk calculator is suggested to reduce the proportion of false-positive cases, with further studies needed for other ethnic groups and complication risk assessment.
