Slower gait speed increases risk of falling in older adults with depression and cognitive complaints

Rebecca K. MacAulay et al.

Summary: Slowed gait is strongly associated with fall risk in older adults, with depression mediating the relationship between gait speed and fall history. The combination of depression and mild cognitive impairments significantly increases the likelihood of falling. This suggests that understanding the underlying pathophysiology of MCI and depression-related gait disturbances could lead to improved intervention strategies for fall risk prevention.

Balance on the Brain: a randomised controlled trial evaluating the effect of a multimodal exercise programme on physical performance, falls, quality of life and cognition for people with mild cognitive impairment-study protocol

Elissa Burton et al.

Summary: This study aims to determine whether a balance-focused multimodal exercise intervention improves balance and reduces falls for people with MCI compared to a control group receiving usual care. A single blind randomised controlled trial involving 396 people with MCI will be conducted to evaluate the primary outcomes of balance performance and rate of falls.

The Effects of Combined Physical and Cognitive Interventions on Direct and Indirect Fall Outcomes for the Elderly with Mild Cognitive Impairment: A Systematic Review

Hai Mai Ba et al.

Summary: This review examined the effectiveness of physical and cognitive training on falls and cognitive function in community-dwelling elderly people with mild cognitive impairment. The findings suggested that PCT significantly impacted the cognitive function and physical activities of elderly people with MCI, particularly improving their balancing ability, gait speed, muscular strength, and executive functions.

Age-Related Cognitive Decline and Prevalence of Mild Cognitive Impairment in the Iwaki Health Promotion Project

Naoko Nakahata et al.

Summary: The study revealed age- and education-related cognitive decline starting from the age of 55, with an estimated prevalence of MCI and dementia at 5.9% in the over 60s in the Iwaki town cohort. Approximately 30% of MCI cases showed progression of cognitive decline.

Volumetric Brain Changes in Older Fallers: A Voxel-Based Morphometric Study

Maxime Le Floch et al.

Summary: Older adults with a history of falls showed larger brain subvolumes in the bilateral striatum, particularly within the caudate nucleus, compared to non-fallers. This suggests a potential brain adaptive mechanism for falls in individuals with neurocognitive decline, especially those with mild cognitive impairment or dementia.

Fall prevention in community-dwelling adults with mild to moderate cognitive impairment: a systematic review and meta-analysis

M. Racey et al.

Summary: This study reviewed fall prevention interventions in community-dwelling older adults with mild to moderate cognitive impairment, finding that exercise interventions are effective in improving balance and fear of falling but the direct impact on reducing falls is still unclear. There is a gap in effective fall prevention interventions for older adults with cognitive impairment, indicating a need for further research in this area.

Diagnosis and Treatment for Mild Cognitive Impairment: A Systematic Review of Clinical Practice Guidelines and Consensus Statements

Ya-Xin Chen et al.

Summary: This study compared and harmonized recommendations for the diagnosis and treatment of Mild Cognitive Impairment (MCI) based on current clinical practice guidelines. Neuropsychological testing and biomarker assessments were common recommendations for MCI diagnosis, while there were differing opinions on pharmacological interventions. Non-pharmacological interventions like physical activity and cognitive interventions were widely recommended. More research is needed on the diagnosis and treatment of MCI.

Mild cognitive impairment, dementia and risk of mortality in essential tremor: A longitudinal prospective study of elders

Nikki Delgado et al.

Summary: In a longitudinal, prospective study of 194 elders with essential tremor (ET), baseline dementia was found to increase the risk of mortality in ET patients by 2-3 times, highlighting the clinical significance of cognitive impairment, particularly dementia, in this population. Various baseline factors, including older age and history of falls, were also independently associated with increased risk of mortality in ET patients.

Summary: In this study, individuals with mild cognitive impairment (MCI) who experienced repeated falls in the previous year had a significantly shorter left one-leg standing time compared to those who did not fall or had only one fall. None of the other physical measures were found to be associated with past repeat falls, including one-leg standing on the dominant right side.
