Pre- and Intraoperative Mapping for Tumors in the Primary Motor Cortex: Decision-Making Process in Surgical Resection

Summary: Background lesions within the primary motor cortex (M1) and the corticospinal tract (CST) present a significant surgical challenge, with the integration of preoperative and intraoperative mapping and histologic information guiding the extent of resection in patients. In this study of 10 patients with lesions within M1 and CST, 70% underwent gross total resection, 20% underwent near-total resection, and only 1 patient was deemed inoperable after mapping. Postoperatively, 70% of patients remained stable and 20% showed improvement in previous motor weakness.

Unsupervised clustering method separated anterior and posterior channels with 96.3% accuracy, automatically delineating the CS location with high precision.
