Impact of malnutrition and frailty on mortality and major amputation in patients with CLTI

Adham M. Karim et al.

Summary: This study aimed to investigate the prevalence of malnutrition and its association with chronic limb-threatening ischemia (CLTI) outcomes, as well as the differential impact of revascularization methods on these outcomes. The results showed that a significant portion of CLTI patients were malnourished or frail, which were associated with higher mortality and amputation rates. Both malnourished and frail patients demonstrated a mortality benefit with less invasive endovascular revascularization methods.

The modified frailty index-11 predicts medium-term outcomes after endovascular revascularisation for chronic limb threatening ischaemia in Asian patients

Shereen X. Y. Soon et al.

Summary: The study evaluated the utility of frailty index (mFI-11) in predicting outcomes for Asian CLTI patients after endovascular revascularization. High frailty was associated with increased 12-month mortality, 30-day complication rate, 30-day unplanned readmission, and worse ambulatory status at 6 and 12 months.

Frailty Is Associated with Impaired Diabetic Foot Ulcer Healing and All-Cause Re-Hospitalization

G. Maltese et al.

Summary: This study aimed to investigate the impact of frailty on the healing and re-hospitalization of patients with Diabetic Foot Ulcers (DFUs) who were hospitalized. The results showed that frail patients had a higher rate of non-healing DFUs and re-hospitalization compared to non-frail patients. Frailty was found to be an independent risk factor for non-healing DFUs and re-hospitalization. Therefore, it is important to assess the frailty status of DFUs patients in order to personalize their management and interventions.

Geriatric Preoperative Optimization: A Review

Kahli E. Zietlow et al.

Summary: This review provides best practices for perioperative care of older adults recommended by major medical societies in the United States. It emphasizes the importance of comprehensive geriatric assessment and rapid screening tools for cognition, functional status, and frailty screening. Providers can use online resources to evaluate polypharmacy and recommend targeted interventions to improve postoperative outcomes for older surgical patients.

Procedural and 12-month in-hospital costs of primary infrapopliteal bypass surgery, infrapopliteal best endovascular treatment, and major lower limb amputation for chronic limb threatening ischemia

Matthew A. Popplewell et al.

Summary: Chronic limb-threatening ischemia (CLTI) is a global issue due to tobacco use and diabetes prevalence. The study compared the costs and effectiveness of primary infrapopliteal vein bypass surgery (BS), best endovascular treatment (BET), and major limb major amputation (MLLA) over a 12-month period, with BET being the most cost-effective option.

Outcomes and Costs of Open and Endovascular Revascularisation for Chronic Limb Ischaemia in an Australian Cohort

Samuel L. Smith et al.

Summary: The study conducted in a single center in Australia analyzed the hospital costs of open and endovascular revascularization for peripheral artery disease, showing that endovascular revascularization had lower costs compared to open revascularization. The total hospital costs decreased over time, mainly due to reduced lengths of hospital stay for open procedures. Readmission within 30 days of discharge was found to be a significant contributor to overall costs.

Associations of Clinical Frailty with Severity of Limb Threat and Outcomes in Chronic Limb-threatening Ischaemia

John S. M. Houghton et al.

Summary: Frailty is highly prevalent among CLTI patients and associated with increased risk of death or amputation, highlighting its importance in risk assessment and management of CLTI patients. The Clinical Frailty Scale may serve as a useful adjunct in evaluating the risk profile of CLTI patients.

How frail is frail? A systematic scoping review and synthesis of high impact studies

E. H. Gordon et al.

Summary: The frailty index (FI) is a continuous variable, but an FI score of 0.25 is commonly used to categorise community-dwelling older adults as frail or non-frail. High impact studies vary in how they categorise the FI, with evidence in the community-dweller literature but not well validated in acute and residential aged care. Reporting FI as a continuous variable is important in these settings for now.
