Primary Versus Staged Repair in Neonates With Pulmonary Atresia and Ventricular Septal Defect

Summary: This study compared the outcomes of two surgical strategies for neonates with ductal-dependent pulmonary atresia and ventricular septal defect. It found that primary biventricular repair approach provides a high survival rate but with a heavy burden of right ventricle outflow tract reintervention, while the staged approach with modified Blalock-Taussig shunt promotes pulmonary artery growth but has significant hospital and interstage mortality. Genetic and extracardiac anomalies are significant risk factors for mortality.
