Simplified Zone 2 Arch Repair Using a Trifurcated Graft for Acute Type A Dissection

Masashi Kai et al.

Summary: We report a simplified zone 2 arch repair technique for acute type A aortic dissection using a trifurcated graft. The right axillary artery is cannulated, and proximal aortic repair is completed using a 1-branched graft. The trifurcated graft is then anastomosed to the ascending graft, and distal aortic anastomosis is performed at the zone 2 level. Unilateral antegrade cerebral perfusion and Y-shaped arterial limbs are used to restore cardiopulmonary bypass flow.

Reoperative Total Arch Repair Using a Trifurcated Graft and Selective Antegrade Cerebral Perfusion

Summary: This study reviews the outcomes of a reoperative total arch repair technique using a trifurcated graft and selective antegrade cerebral perfusion. The results showed that this technique minimizes intervention on cardiac structures, simplifies the surgical procedure, and protects vital organs.

Safe Technique of Coronary Button Preparation in Redo Bentall Operation

Suguru Ohira et al.

Summary: This article introduces a technique of coronary button reconstruction in performing a redo Bentall procedure, which involves preparing a coronary button with a 3 to 4 mm rim of old Dacron graft, sewing it to the new aortic root graft directly or via an interposed graft using a modified Cabrol technique, and anastomosing it directly if the button is comprised of good tissue and well-mobilized.

Predictive factors of distal stent graft-induced new entry after frozen elephant trunk procedure for aortic dissection

Arudo Hiraoka et al.

Summary: This study investigated the incidence and predictors of distal stent graft-induced new entry (d-SINE) after the frozen elephant trunk procedure for aortic dissection. The study found that the incidence of d-SINE was 14.1% and onset time >48 hours was an independent predictor.

Predictive factors of distal stent graft-induced new entry after frozen elephant trunk procedure for aortic dissection

Arudo Hiraoka et al.

Summary: This study investigated the incidence and predictive factors of distal stent graft-induced new entry (d-SINE) after the frozen elephant trunk technique for aortic dissection, finding that onset time >48 h was an independent predictor. The results suggest that preemptive endovascular repair may be appropriate for protecting new entry in high-risk patients.

Comparative outcomes of total arch versus hemiarch repair in acute DeBakey type I aortic dissection: the impact of 21 years of experience

You Jung Ok et al.

Summary: The study evaluated the outcomes of total arch and hemiarch repairs for acute type I aortic dissection over a 21-year period. With the introduction of dedicated aortic surgeons, improvements in surgical strategies led to decreased operative deaths and a reduction in the risk gap between total arch and hemiarch repair.

Valve-sparing aortic root replacement after type A aortic dissection repairs

Noriyuki Abe et al.

Summary: Aortic root reoperation may require aortic root replacement, but a valve-preserving technique should be considered when the aortic cusp is normal. This study showed satisfactory midterm outcomes of reoperative valve-sparing aortic root reconstruction using the reimplantation technique with no early postoperative mortality.
