Tibial shaft fractures in children: flexible intramedullary nailing in growing children especially weighing 50 kg (110 lbs) or more

Dorota Hanf-Osetek et al.

Summary: This study analyzed the indications, complications, and treatment duration of using intramedullary flexible nails (ESIN) in children with displaced tibial shaft fractures who are still growing but weighing 50 kg or more. The results showed that using ESIN in these children is an acceptable and safe treatment method.

Tibial shaft fractures in the adolescents: treatment outcomes and the risk factors for complications

Ahmed M. Thabet et al.

Summary: This retrospective study evaluated the clinical and radiographic outcomes of different treatment methods for adolescent tibial shaft fractures. The results showed no significant differences in union rate and time to fracture union between the different treatment methods. Higher body weight and open fractures were associated with lower union scores. Patients treated with interlocking nailing had higher union scores compared to the casting treatment group. Open fractures had longer union time and higher complication rates.

Observation Versus Cast Treatment of Toddler's Fractures

Joseph Fox et al.

Summary: This study compared the short-term clinical and radiographic outcomes of toddler's fractures treated with long leg casting versus observation. The results showed that observation of toddler's fractures resulted in equivalent clinical and radiographic outcomes, high family satisfaction, and fewer complications compared with treatment with a long leg cast.

Intramedullary Nailing of Paediatric Tibial Fractures: Comparison between Flexible and Rigid Nails

S. Widbom-Kolhanen et al.

Summary: This retrospective study compared outcomes of children undergoing flexible intramedullary nailing with those undergoing rigid nailing for tibial fractures. Results showed that rigid nailing group had better postoperative alignment but higher risk of malunion compared to flexible nailing group.

Lower Leg Fractures in Children and Adolescents-Comparison of Conservative vs. ECMES Treatment

Birte Weber et al.

Summary: Research shows that among pediatric patients, simple oblique fractures of the tibia are the most common lower leg fractures, with half of them being toddler's fractures, primarily treated with conservative therapy. Although the consolidation time was longer in intramedullary nailing (ECMES) compared to conservative therapy, the time to full weight bearing and duration of cast were the same in both groups.

Operative Fixation Versus Cast Immobilization: Tibial Shaft Fractures in Adolescents

Jeffrey E. Martus

Summary: Tibial shaft fractures in adolescents can be managed effectively with closed reduction and cast immobilization. Surgical options such as flexible intramedullary nailing, rigid intramedullary nailing, plate and screw osteosynthesis, and external fixation should be considered for unstable fractures or those that fail conservative treatment. Ongoing observation and timely intervention are crucial to prevent loss of reduction.
