Summary: This study found that the proprotein convertase subtilisin-kexin type 9 inhibitor (PCSK9i) significantly decreased the lipid component of coronary plaques. The combination of statins and PCSK9i in the treatment of coronary artery disease patients may contribute to the stabilization and regression of residual vulnerable plaques.

Current Perspectives on Antithrombotic Therapy for the Treatment of Acute Coronary Syndrome

Summary: Acute coronary syndrome (ACS) is a leading cause of death worldwide, and percutaneous coronary intervention (PCI) is the recommended treatment. However, patients with a history of ACS who undergo PCI are still at high risk for recurrent major adverse cardiac events (MACE). The utilization of antithrombotic drugs reduces MACE rates but can also lead to bleeding complications. Shortening antithrombotic regimens may improve safety, but inappropriate withdrawal can increase the risk of ischemic events. Balancing bleeding risk against the benefits of antithrombotic drugs is challenging, and personalized treatment plans are becoming increasingly important.

Summary: In patients with STEMI undergoing complete revascularization, an FFR-guided strategy did not have a significant benefit over an angiography-guided strategy with respect to the risk of death, myocardial infarction, or urgent revascularization at 1 year. However, the findings do not allow for a conclusive interpretation due to wide confidence intervals.

Summary: Recent studies have shown that in STEMI patients with MVD complicated by cardiogenic shock, CO-PCI resulted in lower risks of renal failure and stroke but increased the risk of myocardial infarction compared to MV-PCI in the short term. No significant difference was observed in long-term follow-up. Staged MV-PCI may be considered in clinical practice to mitigate the risks associated with immediate MV-PCI and CO-PCI, but further large-scale studies are needed to confirm these findings.
