Intelligent and strong robust CVS-LVAD control based on soft-actor-critic algorithm

Te Li et al.

Summary: In this study, a physiological control system for LVAD based on deep reinforcement learning (DRL) is proposed. The experimental result shows that the LVAD based on DRL controller can respond more quickly and effectively to the different physiological needs of a variety of patients than a PID controller.

Pressure and Bernoulli-Based Flow Measurement via a Tapered Inflow VAD Cannula

Kai Petersdorff-Campen et al.

Summary: This study demonstrates the integration of pressure sensors into a ventricular assist device, allowing continuous and accurate measurement of left ventricular pressure. The developed sensor model improves measurement accuracy by eliminating the influence of flow inside the device. This has significant implications for the validation of physiological control concepts in vivo.

The Society of Thoracic Surgeons Intermacs 2020 Annual Report

Ezequiel J. Molina et al.

Summary: The STS-Intermacs 2020 Annual Report analyzed data from 25,551 patients who underwent LVAD implantation between 2010 and 2019, revealing an increase in African American patients and a higher proportion bridged to durable LVAD with temporary mechanical support devices. The report also showed improvements in 1- and 2-year survival rates in the most recent era compared to the previous era, with major bleeding and infection remaining as leading adverse events.

Epidemiology of Heart Failure A Contemporary Perspective

Veronique L. Roger

Summary: This review summarizes recent studies on the epidemiology of heart failure (HF) over the past decade, indicating that while the incidence of HF is mostly flat or declining, the burden of mortality and hospitalization remains significant, especially with major disparities among younger Black men and women. Emerging risk factors and holistic management approaches may help improve the treatment and management of HF.

An Intra-Cycle Optimal Control Framework for Ventricular Assist Devices Based on Atrioventricular Plane Displacement Modeling

Clemens Zeile et al.

Summary: The study proposes an algorithmic framework for constructing and evaluating optimal pump speed policies for LVAD therapy. By reformulating cardiovascular dynamics into a switched system using mathematical optimization, non-linearities are reduced. The findings demonstrate the potential of personalized hemodynamical optimization for LVAD therapy.

Vasoplegia from Continuous Flow Left Ventricular Assist Devices

Shyama Sathianathan et al.

Summary: The role of continuous flow left ventricular assist devices (c-LVAD) in vasoplegic syndrome and postoperative outcomes after orthotopic heart transplant (OHT) is debated in the literature. A standardized definition of vasoplegic syndrome is needed to improve recognition and management of vasoplegic shock. Pulsatility may help mitigate vascular dysfunction caused by long-term continuous flow.
