Summary: Plaque erosion, a distinct entity accounting for over 30% of acute coronary syndromes, can be diagnosed in vivo using optical coherence tomography. Patients with plaque erosion are younger, have fewer cardiovascular risk factors, lower plaque burden, and are more likely to present with non-ST-segment elevation ACS compared to those with plaque rupture. Early evidence suggests that antithrombotic therapy without stenting may be a safe and effective option for ACS caused by plaque erosion.
