Risk of Fracture Among Older Adults With Primary Hyperparathyroidism Receiving Parathyroidectomy vs Nonoperative Management

Carolyn D. Seib et al.

Summary: This study compared the incidence of clinical fracture among older adults with PHPT who underwent parathyroidectomy vs nonoperative management. It found that parathyroidectomy was associated with a lower risk of any fracture and hip fracture, suggesting a clinically meaningful benefit of operative management in this population.

Efficacy of parathyroidectomy compared with active surveillance in patients with mild asymptomatic primary hyperparathyroidism: a systematic review and meta-analysis of randomized-controlled studies

P. Anagnostis et al.

Summary: In patients with mild asymptomatic PHPT, parathyroidectomy (PTx) can increase bone mineral density (BMD) and reduce serum calcium concentrations, but does not show significant superiority over active surveillance in terms of fracture risk, nephrolithiasis, and quality of life (QoL).

Risk of fractures in primary hyperparathyroidism: a systematic review and meta-analysis

Summary: Patients with PHPT have an increased risk of fractures, particularly at the forearm and spine, while the risk of hip fractures is non-significantly increased. The risk of vertebral fractures is also elevated, especially in studies including patients with mild PHPT or postmenopausal women.
