Long-term outcomes after endoscopic endonasal surgery for nonfunctioning pituitary macroadenomas

Summary: Following gross total resection of nonfunctioning pituitary adenomas, the 10-year chance of recurrence is low and the need for treatment even lower. After subtotal resection, upfront radiation therapy may prevent progression, but even without it, the need for intervention at 10 years is only approximately 20%, warranting a period of observation to prevent unnecessary prophylactic radiation therapy. Tumor volume > 1 cm(3), Knosp score >= 3, and Ki-67 >= 3% may prompt closer follow-up or justify early prophylactic radiation therapy.

Nomogram predictive model of post-operative recurrence in non-functioning pituitary adenoma

Summary: This study focused on analyzing and predicting the possibility of post-operative recurrence in NFPA patients by investigating clinical factors leading to tumor recurrence and building a nomogram predictive model based on these risk factors. The results showed that age, tumor size, cavernous invasion, sphenoid sinus invasion, and surgical extension were significant factors affecting tumor recurrence in NFPA patients. The nomogram model developed in this study may assist neurosurgeons in predicting recurrence post-operatively and tailoring counseling for individual patients.

Summary: The Ki-67 index is a useful indicator for predicting proliferation and aggressiveness in pituitary adenomas. This study developed and validated a predictive nomogram model to forecast Ki-67 index levels preoperatively in PAs. The nomogram included age, primary-recurrence subtype, maximum dimension, and prolactin as independent predictors associated with Ki-67 index levels, with stable predictive performance demonstrated in both the training and validation cohorts.
