Randomized Trial Evaluating Clinical Impact of RAPid IDentification and Susceptibility Testing for Gram-negative Bacteremia: RAPIDS-GN

Ritu Banerjee et al.

Summary: The study demonstrated that rapid organism identification and phenotypic AST can lead to faster antibiotic therapy adjustments for patients with gram-negative bloodstream infections (BSIs), ultimately improving treatment outcomes.

Enhanced antimicrobial stewardship based on rapid phenotypic antimicrobial susceptibility testing for bacteraemia in patients with haematological malignancies: a randomized controlled trial

J-H Kim et al.

Summary: The study found that antimicrobial stewardship programmes based on rapid phenotypic AST can significantly increase the proportion of patients with haematological malignancies receiving optimal targeted antibiotics and shorten the time to treatment.

Implementation of a Rapid Phenotypic Susceptibility Platform for Gram-Negative Bloodstream Infections With Paired Antimicrobial Stewardship Intervention: Is the Juice Worth the Squeeze?

Evan D. Robinson et al.

Summary: This study found that implementation of a phenotypic RDT + ASP intervention can reduce the time to antimicrobial susceptibility testing and institutional-preferred antimicrobial therapy for patients with Gram-negative bacilli bloodstream infections. It also showed a decrease in the use of broad-spectrum agents and an increase in narrow-spectrum beta-lactams. Although discrepant RDT results occurred, there were no differences in clinical outcomes observed.

Impact of an Antimicrobial Stewardship Program-bundled initiative utilizing Accelerate Pheno™ system in the management of patients with aerobic Gram-negative bacilli bacteremia

Thomas L. Walsh et al.

Summary: A study was conducted to compare the management of non-critically ill patients with GNB BSI before and after the implementation of a bundled initiative. The initiative included clinical decision algorithm dissemination, ACC testing on all GNB isolated from blood cultures, real-time communication of results to ASP, and prospective audit with feedback by ASP. The implementation led to improvements in time from Gram stain to AST, time to definitive therapy, total duration of antibiotic therapy, and hospital length of stay without increasing readmission rates.

Optimizing the Management of Uncomplicated Gram- Negative Bloodstream Infections: Consensus Guidance Using a Modified Delphi Process

Emily L. Heil et al.

Summary: In the absence of clear guidelines for gram-negative bloodstream infections, a panel of infectious diseases specialists developed a consensus on the management of uncomplicated cases. This included a definition and preferred management approaches, such as a 7-day antibiotic therapy duration, transition to oral antibiotics, and no repeat blood cultures for patients meeting the criteria.
