Radiation necrosis in renal cell carcinoma brain metastases treated with checkpoint inhibitors and radiosurgery: An international multicenter study

Summary: The occurrence of symptomatic radiation necrosis (RN) in patients with renal cell carcinoma (RCC) brain metastases treated with immune checkpoint inhibitors (ICIs) and stereotactic radiosurgery (SRS) is relatively low, with most events being grade 1 to 3 and managed by medical means. Concurrent therapy with ICI/SRS does not appear to increase the risk of RN. Efforts to improve dose conformality may be the most successful strategy for mitigating RN in single-fraction SRS.

Summary: This study investigated the association between neurocognitive outcomes (memory and processing speed) and radiation dose to the hippocampus, corpus callosum, and frontal white matter in children with medulloblastoma. The findings suggest that increasing radiation dose to the corpus callosum and frontal white matter as well as the hippocampus is associated with worse performance on tests of processing speed and associative memory.

Treatment for Brain Metastases: ASCO-SNO-ASTRO Guideline

Summary: The purpose of this study is to provide guidelines for clinicians on treating patients with brain metastases from solid tumors. The study conducted a systematic review of the literature and found that surgery is a reasonable option for patients with brain metastases. Local therapy should be given to patients with symptomatic brain metastases, regardless of the systemic therapy used. For patients with asymptomatic brain metastases, local therapy should not be deferred unless specifically recommended. Several regimens were recommended for different types of cancer. For patients with no systemic therapy options, stereotactic radiosurgery (SRS) alone should be offered. Memantine and hippocampal avoidance should be considered for patients receiving whole brain radiation therapy. Patients with low Karnofsky Performance Status and no systemic therapy options do not benefit from radiation therapy.

Integration of immuno-oncology with stereotactic radiosurgery in the management of brain metastases

Summary: Traditional treatments for brain metastases such as surgery, whole-brain radiation therapy, and glucocorticoids have significant morbidity. However, stereotactic radiosurgery (SRS) has shown to be an effective alternative with improved quality of life when combined with immune checkpoint inhibitors (ICI). Further prospective studies are needed to validate the survival benefits of this combination therapy.

Summary: This study found that patients with brain metastases undergoing hippocampal avoidance whole-brain radiotherapy (HA-WBRT) showed better preservation of memory function at 6-month follow-up compared to those receiving conformal WBRT. However, there were no significant differences in verbal fluency and executive function.
