Understanding the role of WM myelination in LGG development and the migration of glioma cells along subcortical pathways can help tailor individualized therapeutic strategies and optimize long-term oncological and functional outcomes. Further investigation into mechanisms of activity-dependent myelin plasticity is needed for a better understanding of LGG management.

This review discusses current approaches using repetitive transcranial magnetic stimulation (rTMS-NICP) and extraoperative direct cortical stimulation (eDCS-NICP) to induce cortical reorganization before surgery. Preliminary findings suggest that cortical prehabilitation relating to the perisylvian cortex may be effective, but there is a need for further investigation and clarification of optimal stimulation parameters.
