The molecular and cellular mechanisms associated with the destruction of terminal bronchioles in COPD

Summary: This study identifies the molecular and cellular mechanisms associated with peripheral airway obstruction in COPD. It found that the reduction in terminal bronchioles occurs in hot spot regions of microscopic emphysema, where there is upregulation of pro-inflammatory genes and downregulation of inhibitory immune checkpoint genes. There is also an increased infiltration of immune cells in these hot spot regions.

Instead, COPD is now understood as the potential end result of gene-environment interactions accumulated over an individual's lifetime. Future research should aim to understand the dynamic interactions between genes and the environment by integrating information from basic and clinical omics with exposures over time. Additionally, COPD should be viewed as a clinical syndrome rather than a single disease.

In vitro model systems can help understand the functional characteristics of AT2s in homeostasis and disease. The study found that iAT2s cultured at air-liquid interface (ALI) maintained an AT2 phenotype and upregulated expression of transcripts associated with AT2 maturation. The effects of exposure to inhaled toxicants were also assessed.
