Guidelines-similarities and dissimilarities: a systematic review of international clinical practice guidelines for pregnancy hypertension

Georgia Scott et al.

Summary: This study reviewed clinical practice guidelines for pregnancy hypertension and found both consistent recommendations and inconsistencies. Consistent recommendations include automated blood pressure measurement, proteinuria testing, key definitions and classifications, and magnesium sulfate treatment. However, there are inconsistencies in the broad definition of preeclampsia, early identification of preeclampsia risk, the value of calcium in prevention, and other aspects.

The evolution of the diagnostic criteria of preeclampsia-eclampsia

Michael S. Tanner et al.

Summary: With the improved understanding of preeclampsia's pathophysiology, its diagnostic criteria have evolved. This review critically evaluates the evidence that has influenced the development of the criteria, focusing on how they have changed over time and discussing key debates and potential future directions. The importance of considering factors such as age and weight in blood pressure thresholds, the effectiveness of existing blood pressure and proteinuria thresholds, and the potential benefits of using flow-charts and decision trees are among the topics examined.

Maternal whole blood mRNA signatures identify women at risk of early preeclampsia: a longitudinal study

Adi L. Tarca et al.

Summary: Early transcriptomic markers for preeclampsia can be found by analyzing the cellular transcriptome or placental cell-specific mRNAs. The untargeted cellular mRNA signature consistently increased after 22 weeks of gestation, and individual mRNAs from this signature were significantly elevated as early as 11-17 weeks of gestation. Several single-cell placental signatures predicted the future development of the disease at 11-17 weeks and were also increased in women already diagnosed at 32-34 weeks of gestation.

The need for implementation of first trimester screening for preeclampsia and fetal growth restriction in low resource settings

Selvaraj Lakshmy et al.

Summary: The study aimed to evaluate the performance and feasibility of different screening protocols for preeclampsia and fetal growth restriction in low resource settings. Results showed that the multiparametric approach in the first trimester had a higher detection rate of 60% for preeclampsia and 85% for fetal growth restriction, with an even higher detection rate of 98% for early onset preeclampsia in certain cases. Implementing screening for preeclampsia with available resources in peripheral areas is crucial to prevent severe complications, and a multiparametric approach in the first trimester could be an effective strategy in low resource settings.

Summary: The study reveals tissue-specific and position-dependent differences in expression levels of C19MC miRNA cluster in embryonic stem cells and placenta, possibly due to differences in posttranscriptional processing. In placenta, there are enhancer regions associated with the Microprocessor complex, leading to more efficient processing of pri-miRNA.
