Early endoscopic treatment of symptomatic pancreatic necrotic collections

Summary: The rapid development of advanced gastrointestinal endoscopic techniques has led to the emergence of new biomedical materials, such as polymers, used for producing various types of endoprostheses. The high efficacy of endotherapy in treating postinflammatory pancreatic and peripancreatic fluid collections is associated with potential complications related to the design of the lumen-apposing metal stent used. Therefore, further exploration of new technological directions is needed to increase the safety of treatment.

Laparoscopic transgastric necrosectomy in treatment of walled-off pancreatic necrosis with sinistral portal hypertension

Summary: This study retrospectively analyzed WON patients with sinistral portal hypertension treated in the department between January 2011 and December 2018. The results showed that LTGN was associated with lower morbidity and similar severe complication rates compared to LATLSN. There were no significant differences in postoperative hemorrhage, mortality, WON recurrence rate, or long-term complications between the two groups after long-term follow-up.

Percutaneous endoscopic necrosectomy seems promising before emergency surgery, but should be reserved in referral centers with a multidisciplinary team.
