A first-in-human clinical study of a new SP-B and SP-C enriched synthetic surfactant (CHF5633) in preterm babies with respiratory distress syndrome: two-year outcomes

David G. Sweet et al.

Summary: This study assessed the neurological, respiratory, and general health status of premature infants treated with a new synthetic surfactant enriched with SP-B and SP-C proteins at 24 months corrected age. The results showed that the surfactant treatment was not associated with adverse neurodevelopmental, respiratory, or health outcomes by two years of age.

State-of-the-art review of the application and development of various methods of aerosol therapy

M. Matuszak et al.

Summary: Aerosol therapy is an increasingly popular method of drug administration due to its advantages over other methods such as injection and oral methods. Understanding the basic principles and applications of aerosol therapy is crucial for effective treatment. Factors such as the physicochemical properties of the sprayed system, the design and correct application of medical inhalers, and inhalation dynamics play a significant role in the effectiveness of inhalation. Respiratory system diseases are prevalent and growing rapidly, leading to a rise in the availability of new spraying devices and pharmaceutical drugs. Analyzing the characteristics of aerosol droplets is essential in determining the effectiveness of the atomization process and drug delivery to the respiratory tract. The presented literature review provides valuable insights for understanding the spraying process and selecting appropriate medical inhalers for liquids with specific rheological properties.

Characterizing the Effects of Nasal Prong Interfaces on Aerosol Deposition in a Preterm Infant Nasal Model

Karl Bass et al.

Summary: The objective of this study was to investigate the effects of different nasal prong configurations on nostril deposition of pharmaceutical aerosols in preterm infants. The results showed that the use of nasal prongs significantly affected the deposition of aerosols in the infant nasal cavity, calling for design alterations to improve lung delivery efficiency.

Summary: This study investigated the feasibility, safety, and efficacy of four dosing schedules of aerosolized surfactant in preterm infants with respiratory distress syndrome. The results showed that this treatment was well tolerated, had short-term efficacy, and infants who received aerosolized surfactant were less likely to require intubation within 72 hours compared to historical controls.

Structural and functional stability of the sulfur-free surfactant protein B peptide mimic B-YL in synthetic surfactant lipids

Frans J. Walther et al.

Summary: The study confirmed the structural and functional stability of the B-YL peptide in synthetic surfactant lipids after three years of storage at 5 degrees C, with no degradation observed in mass spectrometry analysis and stable alpha-helical conformations identified in ATR-FTIR studies. In vitro surface activity on captive bubble surfactometry and in vivo surface activity in surfactant-deficient young adult rabbits were excellent after the prolonged storage period, indicating that the B-YL surfactant maintained its integrity and functionality over time.

Development and Characterization of Excipient Enhanced Growth (EEG) Surfactant Powder Formulations for Treating Neonatal Respiratory Distress Syndrome

Susan Boc et al.

Summary: This study developed and characterized a spray-dried powder aerosol formulation of a commercially available surfactant formulation, Survanta (R) intratracheal suspension, using the excipient enhanced growth (EEG) approach. The inclusion of l-leucine in the formulation significantly reduced primary particle size and increased the fraction of submicrometer particles, resulting in good aerosolization performance with low moisture content.

Physiologic Effects of Instilled and Aerosolized Surfactant Using a Breath-Synchronized Nebulizer on Surfactant-Deficient Rabbits

Robert M. DiBlasi et al.

Summary: The study compared the efficacy and safety of surfactant administration via aerosolization and liquid bolus instillation, demonstrating that aerosolized surfactant had similar improvement in oxygenation response and greater ventilation efficiency compared to liquid bolus administration, with fewer peridosing events. This suggests that breath-synchronized aerosol via nebulizer could be a potential safe, effective, and economical alternative to liquid surfactant instillation.

Aerosol drug delivery to spontaneously-breathing preterm neonates: lessons learned

Federico Bianco et al.

Summary: The delivery of medications to preterm neonates receiving non-invasive ventilation (NIV) is a challenging scenario for aerosol medicine due to anatomical and physiological characteristics of the lungs in such infants. Factors such as low lung volumes, compliance, and irregular respiratory rates affect lung deposition. Extrinsic factors like aerosol generator choice, drug formulation, particle size distribution, NIV type, and patient interface also impact lung deposition and offer an opportunity to optimize therapy efficacy. Existing experimental and clinical data on neonatal aerosol delivery are examined, with a focus on non-invasive delivery of surfactant aerosols, providing insights and a framework for the development of inhaled drugs in this patient population.
