A Novel Scoring System for the Risk of Papillary Thyroid Cancer Metastases in Lymph Nodes Posterior to the Right of the Recurrent Laryngeal Nerve

Summary: In this study, a predictive model based on risk factors was established to help determine the necessity of PRRLN-LN dissection for patients with papillary thyroid carcinoma. Age, tumor size, extrathyroidal extension, clinically involved right central compartment lymph node metastasis, and right lateral lymph node metastasis were identified as predictors of LN-prRLN. The established risk model was well validated with a concordance index of 0.756 +/- 0.058 and 0.745 +/- 0.042 in the development and validation cohorts, respectively.

Summary: The study aimed to investigate the correlation between PTC characteristics on ultrasonography and LN-prRLN metastases, and establish a scoring system to determine which patients with PTC require LN-prRLN resection. A prediction model was developed based on risk factors, and preoperative clinical data were found to be positively correlated with LN-prRLN metastases. The scoring system can help surgeons determine which patients should undergo LN-prRLN resection, with a recommended cutoff score of 103.1.
