Predicting atrial fibrillation after cryptogenic stroke via a clinical risk score-a prospective observational study

Markus Kneihsl et al.

Summary: A clinical risk score was developed for predicting atrial fibrillation (AF) occurrence in cryptogenic stroke (CS) patients, demonstrating high sensitivity, reasonable specificity, and excellent negative predictive value over a 1-year follow-up period. Further validation in external cohorts with continuous cardiac rhythm monitoring is needed to assess the generalizability of the score.

Identification of patients with embolic stroke of undetermined source and low risk of new incident atrial fibrillation: The AF-ESUS score

George Ntaios et al.

Summary: The study developed a scoring system to identify ESUS patients with low risk of new incident atrial fibrillation (AF). The proposed scoring system has high sensitivity and negative predictive value, with patients scoring <= 0 being at low risk of developing new AF during follow-up.

Effect of Long-term Continuous Cardiac Monitoring vs Usual Care on Detection of Atrial Fibrillation in Patients With Stroke Attributed to Large- or Small-Vessel Disease The STROKE-AF Randomized Clinical Trial

Richard A. Bernstein et al.

Summary: The STROKE-AF trial demonstrated that among patients with stroke attributed to large- or small-vessel disease, long-term cardiac monitoring with an insertable cardiac monitor (ICM) detected significantly more cases of atrial fibrillation (AF) over 12 months compared to usual care. However, further research is needed to determine the clinical significance of identifying AF in these patients.

Biomarkers for Atrial Fibrillation Detection After Stroke Systematic Review and Meta-analysis

Alan Cameron et al.

Summary: This study identified various clinical, ECG, and blood-based biomarkers associated with atrial fibrillation (AF) detection after ischemic stroke or TIA. Factors such as older age, female sex, history of heart failure, hypertension, and certain ECG parameters were found to be associated with increased likelihood of AF detection, while other factors like certain lipid concentrations were associated with reduced likelihood. These multimodal biomarkers could potentially be used to guide patient selection for cardiac monitoring after ischemic stroke/TIA, but further prospective assessments are needed to confirm their prognostic utility.

Predicting Atrial Fibrillation with High Risk of Embolization with Atrial Strain and NT-proBNP

Jorge Pagola et al.

Summary: The study aimed to determine markers of atrial dysfunction in patients with cryptogenic stroke to predict episodes of paroxysmal atrial fibrillation with high risk of embolization. The results showed that the combination of age and PALS, NT-proBNP can independently predict paroxysmal atrial fibrillation, and the validity of PALS and NT-proBNP in detecting patients at risk of HpAF was higher than age.
