Summary: Cerebral venous thrombosis (CVT) is a rare cerebrovascular disease where inflammation plays a crucial role. Inflammatory factors worsen brain tissue injury and lead to poor prognosis. Peripheral blood inflammatory biomarkers, especially the neutrophil-to-lymphocyte ratio (NLR) and lymphocyte count, are correlated with CVT and predict patient outcomes. COVID-19 infection and vaccine can also induce CVT through inflammatory reactions.

The overall incidence of CVT was found to be similar to that reported in the Netherlands and Australia, with a 5.0% annual increase in admissions and low in-hospital mortality. Sex-specific incidence rates differed significantly between age groups, and long-term mortality was higher in older patients and men.

Genetic polymorphisms of the MTHFR gene are associated with CVST, along with other factors such as local infections, general infections, obstetric causes, and head injury. The study found a significant correlation between MTHFR polymorphisms and higher cholesterol levels, low-density lipoprotein cholesterol, and homocysteine levels. High-sensitive C-reactive protein levels and hyperhomocysteinemia were also identified as potential prognostic factors in CVST.

Identifying biomarkers associated with venous infarction in CVT may help understand the pathogenesis and provide potentially useful therapeutic markers. Serum hs-CRP levels were significantly higher in acute/subacute CVT patients with venous infarction than those without, and hs-CRP was positively correlated with the NIHSS on admission. Hs-CRP might serve as a biomarker reflecting the clinical severity of CVT in the acute/subacute stages.
