Summary: This study suggests that bacterial infection can cause sensitization of bladder sensory neurons, leading to bladder overactivity and pelvic pain.

Effect of antibiotics on urine leakage in women with refractory detrusor overactivity: A phase IIb randomized trial

Zhuoran Chen et al.

Summary: The study aimed to compare the efficacy of rotating antibiotics versus placebo in women with refractory detrusor overactivity, in conjunction with an anticholinergic. Results showed a significant reduction in pad leakage and leaks per day over 6 months in the antibiotic group, with nearly half of patients on placebo requiring clinical override for disabling urinary tract infection symptoms.

Urinary cytokines in women with refractory detrusor overactivity: A longitudinal study of rotating antibiotic versus placebo treatment

Zhuoran Chen et al.

Summary: The study investigated the impact of bacteriuria and antibiotic treatment on urinary cytokine levels in women with refractory detrusor overactivity. Results revealed that the presence of bacteriuria increased levels of certain cytokines, while antibiotic treatment reduced the release of these cytokines and was associated with symptom improvement. This suggests that inflammatory response may play a role in the etiology of refractory detrusor overactivity.

Summary: Women with symptoms of overactive bladder often have significant bacterial growth in enhanced urine cultures, not detectable through routine culture, suggesting the presence of subclinical infection. Enhanced culture techniques should be routinely used for the effective diagnosis and management of overactive bladder.

Summary: The meta-analysis revealed that trigone-including BoNT-A injection was more effective than trigone-sparing injection for OAB patients, showing improvements in symptom scores, complete dryness rates, frequency of incontinence episodes, detrusor pressure at maximum flow rate, and volume at the first desire to void, especially beneficial for NDO patients.

Summary: The study revealed significant differences in urine IL6 and lactoferrin between OAB patients and controls, with clinical implications related to symptoms and pyuria. These pathological markers were missed by standard screening methods, indicating the need for reevaluation of UTI in OAB patients.
