Study of the Involvement of the P2Y12 Receptor in Chronic Itching in Type 2 Diabetes Mellitus

Xiumei Xu et al.

Summary: This study investigated the pathological changes in the P2Y12 receptor in type 2 diabetes mellitus complicated with chronic itching. The results showed that the upregulation of P2Y12 receptor expression was closely associated with increased ROS levels, increased release of inflammatory cytokines, and nerve damage.

GPR177 in A-fiber sensory neurons drives diabetic neuropathic pain via WNT-mediated TRPV1 activation

Ya-Kai Xie et al.

Summary: This study reveals the mechanisms underlying the development of DNP in patients with diabetes and identifies a potential therapeutic target. The researchers found that GPR177 in A-fiber neurons mediates the secretion of WNT5a, which in turn activates the TRPV1 ion channel and causes pain. These findings contribute to the search for therapies to relieve neuropathic pain.

Painful and non-painful diabetic neuropathy, diagnostic challenges and implications for future management

Troels S. Jensen et al.

Summary: Peripheral neuropathy is a common complication of diabetes, with the most common type being distal symmetric polyneuropathy. Diagnosis of diabetic neuropathy remains challenging due to lack of a single established diagnostic standard, leading to a reliance on a hierarchical system combining symptoms, signs, and confirmatory tests for research purposes. Treatments for diabetic neuropathy focus on glycemic control and symptomatic management, with a need for large randomized clinical trials to identify the most effective interventions for improved patient outcomes.

Summary: In this study, itch was found to be a relatively common symptom in patients with type 2 diabetes mellitus, impacting their quality of life significantly. The primary cause of itch in these patients appears to be poor diabetes control leading to skin dryness and diabetic neuropathy.

Spinal glial cells in itch modulation

Miho Shiratori-Hayashi et al.

Summary: Glial cells, particularly astrocytes, have been found to play a significant role in modulating itch processing in the nervous system. Their activation in response to skin inflammation contributes to the exacerbation of chronic itch. Studies have highlighted the importance of glial cells in the pathogenesis and progression of neurological diseases.

Aging and the immune response in diabetic peripheral neuropathy

Kathleen M. Hagen et al.

Summary: A large proportion of older individuals with diabetes develop diabetic peripheral neuropathy (DPN), which is associated with inflammatory cells increase, oxidative stress, proinflammatory changes in the bone marrow, among other factors, all of which may be exacerbated with age. The immunological features of DPN and age-related changes in relevant immunological areas may contribute to age being a risk factor for DPN.

Painful Diabetic Peripheral Neuropathy Study of Chinese Outpatients (PDNSCOPE): A Multicentre Cross-Sectional Registry Study of Clinical Characteristics and Treatment in Mainland China

Yuanjin Zhang et al.

Summary: This study investigated the current clinical scenarios of painful diabetic peripheral neuropathy and associated anxiety and depression among patients in Mainland China. The majority of participants experienced moderate to severe neuropathic pain. Age, diabetic duration, depression history, PHQ-9 and GAD-7 scores, and FBG were identified as risk factors for VAS pain scores, while PAD was found to be a protective factor. Most prescribed pain relief therapies were in line with guidelines.

Summary: Research findings suggest that the epidemiology of itch in diabetes ranges from 18.4% to 27.5%, with the main factors being skin xerosis and diabetic polyneuropathy. Currently, there is no preferred treatment for diabetes-related itch, but topical therapy shows significant relief in some patients.
