The primary outcome is the distribution of 90-day mRS scores, with secondary outcomes including functional independence rate, safety, and imaging outcomes. The SELECT2 trial may provide randomized evidence to expand endovascular thrombectomy eligibility to a larger population.

Summary: Limited data are available regarding the outcomes of mechanical thrombectomy (MT) for real-world stroke patients with a large core infarct. This study aimed to investigate the safety and effectiveness of MT for patients with large vessel occlusion and an Alberta Stroke Program Early Computed Tomography Score (ASPECTS) of 2 to 5. In a cohort of 2345 patients, more than 1 in 5 patients with an ASPECTS of 2 to 5 achieved 90-day functional independence after MT.

Reversible Ischemic Lesion Hypodensity in Acute Stroke CT Following Endovascular Reperfusion

Summary: In a study of 184 patients with acute ischemic anterior circulation stroke, it was found that a small percentage exhibited reversible ischemic edema after complete recanalization. Factors associated with edema arrest or reversibility included higher degree of recanalization and shorter time from imaging to recanalization. Clinical outcome was significantly better in patients without edema progression. This suggests that early revascularization may halt or even reverse vasogenic edema of ischemic tissue, leading to improved functional outcomes.
