Summary: CLL rarely presents in AYA patients aged 15-39 years. Disease characteristics and outcomes of AYA patients are not well understood. Analysis of 227 AYA patients diagnosed in the last two decades showed median TTFT of 2.2 years and five- and 10-year OS of 90% and 78%, respectively. Pre-treatment factors and genetic mutations were associated with shorter TTFT and OS in AYA CLL patients. Within the subgroup receiving oral targeted therapy, specific genetic mutations were associated with shorter OS. Age was significantly associated with OS and time to Richter transformation.
