Summary: The trial involving 10,584 patients with diabetes and chronic kidney disease showed that sotagliflozin resulted in fewer deaths from cardiovascular causes, hospitalizations for heart failure, and urgent visits for heart failure compared to placebo. However, there were adverse events such as diarrhea, mycotic infections, and diabetic ketoacidosis associated with sotagliflozin.

Multiple agents used in combination are likely to be needed to effectively slow the progression of the disease.

Complement factor B in high glucose- induced podocyte injury and diabetic kidney disease

Summary: In diabetic kidney disease (DKD), upregulation of complement factor B (CFB) in podocytes is mediated through mTORC1 signaling, leading to podocyte injury. Blocking mTORC1 signaling can reduce CFB upregulation and podocyte injury. Knocking down CFB alleviates alternative complement pathway activation in diabetic mice and cultured podocytes. High glucose treatment activates mTORC1 signaling and upregulates CFB expression, while blocking mTORC1 or STAT1 signaling can counteract this effect and prevent podocyte injury. Additionally, downregulation of protein phosphatase 2Ac alpha (PP2Ac alpha) enhances mTORC1/STAT1 activation, CFB induction, and podocyte injury in response to high glucose levels.
