Effectiveness and Safety of EUS Rendezvous After Failed Biliary Cannulation With ERCP A Systematic Review and Proportion Meta-analysis

Jagpal S. Klair et al.

Summary: A systematic review and meta-analysis evaluated the effectiveness and safety of endoscopic ultrasound-guided rendezvous ERCP in patients who failed biliary cannulation with conventional ERCP. The results demonstrated high rates of technical and clinical success with EUS-RV-ERCP, but also indicated a risk of adverse events.

Summary: ESGE recommends various endoscopic ultrasound-guided interventions for different biliary, pancreatic, gastric, and gallbladder diseases with different levels of evidence quality, highlighting the strong recommendation for EUS-GBD in high surgical risk patients due to its lower rates of adverse events and need for re-interventions.

EUS-guided biliary drainage with electrocautery-enhanced lumen-apposing metal stent placement should replace PTBD after ERCP failure in patients with distal tumoral biliary obstruction: a large real-life study

C. Ginestet et al.

Summary: This study compared the real-life efficacies of percutaneous transhepatic biliary drainage (PTBD) and endoscopic ultrasound-guided biliary drainage (EUS-BD) with electrocautery-enhanced lumen-apposing metal stent (EC-LAMS) in cases of ERCP failure for malignant biliary obstruction. The results showed that EUS-BD with EC-LAMS had higher clinical success rates, safety, cost-effectiveness, and overall survival compared to PTBD in this patient population.

Safety and efficacy of percutaneous transhepatic-endoscopic rendezvous procedure in a single session

Cyril Chivot et al.

Summary: The technical success rate of PTE-RV was 95.2%, but 19% of cases experienced adverse events with a 9.5% mortality rate within 30 days post-procedure. The study demonstrates that performing PTE-RV in a single session is feasible with an acceptable level of risk.

Endoscopic ultrasound-guided biliary drainage-current status and future perspectives

Petko Ivanov Karagyozov et al.

Summary: ERCP with stenting is the preferred treatment for bile duct obstruction, but may fail in certain cases. Traditional rescue procedure PTBD is associated with high risk, while EUS-BD, a relatively new intervention, has shown effectiveness and potential to become a primary treatment option in the future.

Technical Review of Developments in Endoscopic Ultrasound-Guided Hepaticogastrostomy

Takeshi Ogura et al.

Summary: Endoscopic ultrasound-guided hepaticogastrostomy is a technically complex alternative method for biliary drainage, with challenges including difficulty in puncturing the intrahepatic bile duct and risk of stent migration. Recent advances in technique and devices have improved the success rate and reduced adverse events, but endoscopists should remain cautious about critical complications.

Management of biliary diseases after the failure of initial needle knife precut sphincterotomy for biliary cannulation

Min-Hao Lo et al.

Summary: After initial failed NKPS, IET should be considered for deep biliary cannulation before more invasive treatments like BDS. PTBD may serve as an alternative therapy for patients with malignant biliary obstruction. Complications were lowest in the IET group and mild compared to the other two groups.
