Semimechanistic Pharmacokinetic and Pharmacodynamic Modeling of Piperaquine in a Volunteer Infection Study with Plasmodium falciparum Blood-Stage Malaria

Summary: Dihydroartemisinin-piperaquine is commonly used as a first-line treatment for Plasmodium falciparum malaria, and this study aimed to develop a pharmacokinetic-pharmacodynamic model to optimize its use in new antimalarial combination therapies. By analyzing data from healthy volunteers, the study characterized the pharmacokinetic properties of piperaquine and developed a semimechanistic parasite dynamics model related to piperaquine exposure. Treatment simulations suggested that an ideal additional drug in new antimalarial triple-combination therapies should have specific parasite reduction ratios and durations to combat multidrug-resistant infections effectively.

Although artemether-lumefantrine remains efficacious, vigilance for decreasing efficacy and further characterization of artemisinin partial resistance should be considered. Evaluation of additional antimalarials in Rwanda is also recommended.

The cardiovascular effects of amodiaquine and structurally related antimalarials: An individual patient data meta-analysis

Summary: Amodiaquine, a common antimalarial drug, has been found to have mild effects on cardiac electrophysiology, including prolonging the QT interval and inducing sinus bradycardia. Compared to other antimalarials, amodiaquine has shown different impacts on heart rate and QT interval, but no serious cardiovascular events have been reported after over seven decades of widespread use.
