Summary: Early head computed tomography (HCT) within 6 hours of cardiac arrest (CA) can impact therapeutic decisions, with 25% of cases showing abnormalities and nearly half resulting in management changes. Cases with radiographic hypoxic-ischemic brain injury (HIBI) had lower gray-white matter ratio (GWR) and higher progression to brain death. Interpretation discrepancies between radiology and neurointensivists were common and agreement on severity of HIBI on early HCT was poor.
