International recommendations for personalised selective internal radiation therapy of primary and metastatic liver diseases with yttrium-90 resin microspheres

Hugo Levillain et al.

Summary: This study established recommendations for the optimisation of selective internal radiation therapy (SIRT) with yttrium-90 (Y-90)-resin microspheres, based on expert panel discussions and survey responses from leading experts in the field. The recommendations cover various aspects of SIRT, including activity prescription, post-treatment dosimetry, and imaging for treatment verification. It is recommended that a multidisciplinary team define treatment strategy and therapeutic intent, and that a personalised approach using dosimetry be adopted for activity prescription. Additionally, post-SIRT imaging and dosimetry are strongly recommended for treatment verification and assessment.

Lung shunt and lung dose calculation methods for radioembolization treatment planning

S. Cheenu Kappadath et al.

Summary: Radioembolization, also known as selective internal radiation therapy (SIRT), is an established treatment for unresectable liver cancers, but lung dosimetry still faces challenges. The current methods for lung dose estimation suffer from poor accuracy and precision. Further research and improvements are needed to standardize lung dose calculations for routine clinical practice.

Systematic review of lung shunt fraction quantification comparing SPECT/CT and planar scintigraphy for yttrium 90 radioembolization planning

Harkanwar Gill et al.

Summary: Yttrium 90-labeled intra-arterial liver therapy is effective for patients with liver malignancy. 3D SPECT/CT provides more accurate measurements of lung shunt fraction (LSF) compared to 2D planar scintigraphy, with a narrower range of LSF percentages. However, further studies are needed to address the quality concerns related to blinding between the index test and reference standard.

EANM dosimetry committee series on standard operational procedures: a unified methodology for 99mTc-MAA pre- and 90Y peri-therapy dosimetry in liver radioembolization with 90Y microspheres

Carlo Chiesa et al.

Summary: The standard operational procedure aims to standardize the evaluation methodology for absorbed dose calculations in Y-90 microsphere liver radioembolization. Treatment planning based on average absorbed dose is advised, with distinct evaluation between target tumors and non-tumoral tissue being vital for proper therapy optimization. Various imaging methodologies such as SPECT/CT and PET/CT are recommended for accurate dosimetry calculations.

Accurate non-tumoral 99mTc-MAA absorbed dose prediction to plan optimized activities in liver radioembolization using resin microspheres

Philippe d'Abadie et al.

Summary: This study evaluates dose parameters in liver radioembolization using MAA SPECT/CT imaging and proposes a prescription of activity based on MAA distribution in the normal liver. Results show accurate prediction of whole normal liver absorbed doses with MAA imaging, indicating potential for optimizing activity planning. Activities calculated using MAA distribution in the normal liver compartment were significantly higher compared to those defined with the BSA method.
