Lung microbiota predict chronic rejection in healthy lung transplant recipients: a prospective cohort study

Summary: The characteristics of the respiratory microbiome in healthy lung transplant recipients within the first year of transplantation have a significant impact on subsequent CLAD-free survival, with an increased lung bacterial burden predicting chronic rejection and death. The lung microbiome represents an understudied and potentially modifiable risk factor for lung allograft dysfunction.

Summary: Bronchiectasis patients at greater risk of exacerbation exhibit less complex microbial co-occurrence networks, reduced diversity, and higher levels of antagonistic interactions in their airway microbiome. Incorporating microbial interaction data improves clinical prediction models for exacerbation risk. Antimicrobial strategies could potentially target interaction networks rather than individual microbes, offering a new approach to understanding respiratory infections.

Summary: This study examined the viable bacterial community in bronchoalveolar lavage samples from 64 lung transplant recipients and identified four distinct compositional states, called "pneumotypes," which were associated with immune activity, lung function, and risks of infection and rejection.
