The updated 2021 focused update highlighted issues such as the AF Better Care pathway, advantages of NOACs for Asians, and the use of oral anticoagulants during the COVID-19 pandemic. While acknowledging gaps and uncertainties in current knowledge of AF, the importance of physician's decision-making in AF management was emphasized.

Gut microbiota dysbiosis promotes age-related atrial fibrillation by lipopolysaccharide and glucose-induced activation of NLRP3-inflammasome

Summary: By using a faecal microbiota transplantation (FMT) rat model, it was demonstrated that the high AF susceptibility of aged rats could be transmitted to a young host. Inhibition of inflammasome by a potent and selective inhibitor of the NLRP3 inflammasome resulted in a lower atrial fibrosis and AF susceptibility. Clinical studies showed that serum LPS and glucose levels were progressively increased in elderly patients compared to young individuals, with similar results confirmed in clinical AF patients.

Summary: A systematic review and meta-analysis of studies on the use of the ABC pathway in patients with atrial fibrillation showed suboptimal adherence at 21%, but adherence was associated with a lower risk of all-cause death, cardiovascular death, stroke, and major bleeding. Comorbidities and longer follow-up were identified as factors affecting heterogeneity and effectiveness of the pathway.
