Reclassification of patients with tremor syndrome and comparisons of essential tremor and essential tremor-plus patients

Jiaxin Peng et al.

Summary: This study reclassified and compared the characteristics of ET and ET-plus patients in a large Chinese tremor cohort. The results showed that ET-plus patients were older, had longer disease durations, worse tremor manifestations, and more distinct non-motor symptoms. Certain additional soft signs of ET-plus were associated with tremor severity or worse quality of life.

Parkinsonism and tremor syndromes

Steven Bellows et al.

Summary: Tremor is a common movement disorder that can occur alone or coexist with other neurological and movement disorders. It can be a symptom of Parkinson's disease, dystonia, ataxia, and other disorders. Essential tremor may not only be associated with Parkinson's disease but also with other neurological disorders, suggesting the existence of different subtypes. Besides Parkinson's disease, tremor can also be a prominent feature in other parkinsonian disorders and movement disorders.

The Italian tremor Network (TITAN): rationale, design and preliminary findings

Roberto Erro et al.

Summary: The TITAN study is a multicenter data collection platform aimed at assessing the phenomenology and natural history of tremor syndromes. The study found that the frequency and characteristics of tremor syndromes vary. Patients with ET-plus were older and had a higher age at onset but a shorter disease duration compared to ET, suggesting that ET-plus is not a disease stage of ET.

The differences in clinical characteristics and natural history between essential tremor and essential tremor plus

Praween Lolekha et al.

Summary: The different phenotypes of essential tremor (ET) may affect the clinical outcome and progression. This study found that patients with ET-plus had older age of onset, higher frequency of cranial tremors, neurological and psychiatric comorbidities, more tremor progression, and poorer response to medical treatment compared to ET patients. Identifying the ET phenotypes is important for future therapeutic strategies and clinical research.

Essential tremor plus rest tremor: current concepts and controversies

Roberto Erro et al.

Summary: This article systematically reviews studies on ET-plus rest tremor and concludes that it is a heterogeneous group of conditions, potentially related to late-stage ET, different disorders, the development of Parkinson's disease, or misdiagnosis of ET. The study highlights the need for further research to solve methodological issues.

Features of ET plus correlate with age and tremor duration: ET plus may be a disease stage rather than a subtype of essential tremor

Elan D. Louis et al.

Summary: As patients with essential tremor (ET) age and the duration of tremor increases, they tend to develop more features of ET plus, which may represent a disease stage rather than a distinct subtype of the disease.

Summary: This study found no significant pathological differences in cerebellar cortex between cases of ET and ET-plus, thus not supporting the idea that ET and ET-plus represent distinct clinical-pathological entities.

Abnormalities of intrinsic brain activity in essential tremor: A meta-analysis of resting-state functional imaging

Summary: Neuroimaging studies have shown abnormal patterns of brain activity in patients with ET in areas including the cerebellum, motor cortex, prefrontal cortex, and insula. These consistent findings contribute to a better understanding of the pathophysiology of ET.

Voxel-Based Morphometry of Cerebellar Lobules in Essential Tremor

Richard Agren et al.

Summary: Comparison of cerebellar lobule volumes between ET patients and HCs using automated segmentation revealed smaller volumes in ET patients, opposing the hypothesis of localized atrophy in cerebellar motor areas in ET. However, the possibility of cerebellar pathophysiology in ET was not ruled out. Further prospective investigations using alternative neuroimaging modalities may help elucidate the pathophysiology of ET and guide diagnostic and therapeutic approaches.
