Prevalence of faecal carriage of Carbapenemase Producing Enterobacteriaceae in healthy Indian subjects from the community

Nasheed Arum et al.

Summary: This study investigated the carriage rate of carbapenemase-producing Enterobacterales (CPE) in healthy individuals in India, and found a carriage rate of 6.1%. Most of the isolates displayed susceptibility to imipenem, but carbapenemase-producing isolates were still present. Comprehensive measures to improve sanitation and implementation of national antimicrobial resistance action plans are needed to prevent further generation and dissemination of carbapenem-resistant Enterobacterales (CRE).

Association Between Types of Carbapenemase and Clinical Outcomes of Infection Due to Carbapenem Resistance Enterobacterales

Korawan Pudpong et al.

Summary: This study aimed to investigate the impact of carbapenemase gene types on clinical outcomes of carbapenem-resistant Enterobacterales (CRE) infection. The results showed that patients infected with NDM-1 or NDM-1/OXA-48 had a higher mortality rate compared to those infected with OXA-48, and the type of carbapenemase gene may affect the minimum inhibitory concentration (MIC) of meropenem. Therefore, close monitoring of patient symptoms is necessary when deciding on treatment options, especially in the NDM-1 and NDM-1/OXA-48 groups, for CRE infection.

Rescuing the Last-Line Polymyxins: Achievements and Challenges

Sue C. Nang et al.

Summary: Antibiotic resistance is a major global health challenge, and polymyxins have been revived as a last-line therapeutic option for infections caused by multidrug-resistant Gram negative bacteria. Recent efforts have been made to elucidate the properties of polymyxins and develop scientifically based dosage regimens to ensure safe and effective use. Challenges remain in optimizing clinical use and discovering new-generation polymyxins to overcome toxicities and narrow therapeutic windows.

Four Types of ST11 Novel Mutations From Increasing Carbapenem-Resistant Klebsiella pneumoniae in Guangdong, 2016-2020

Yunhu Zhao et al.

Summary: The infection rate of CR-KP has increased, with ICU and neonatal wards as key infection areas. Serine enzyme, KPC genotype, and ST11 are predominant CR-KP types. Polygenic strains and ST11 mutations pose challenges to clinical treatment and potential threats.

Clinical and Microbiological Characterization of Carbapenem-Resistant Enterobacteriales: A Prospective Cohort Study

Qiuxia Lin et al.

Summary: In Chongqing, China, the study revealed that CRE infections were associated with high mortality rates and poor outcomes, with CPE patients more likely to exhibit antibiotic resistance. Independent predictors for CPE infection were ICU admission and hepatobiliary system diseases. Although there was no significant difference in desirability of outcome ranking (DOOR) outcomes between the two groups, CPE patients had prolonged ICU stays.

Clonal spread of carbapenem-resistant Klebsiella pneumoniae among patients at admission and discharge at a Vietnamese neonatal intensive care unit

Bjorn Berglund et al.

Summary: The study in a Vietnamese neonatal intensive care unit revealed highly resistant carbapenem-resistant Klebsiella pneumoniae (CRKP) colonizing both admission and discharge patients, predominantly consisting of ST15 isolates. These isolates carried NDM-type and KPC-2 resistance genes, forming three distinct clonal clusters. The research also showed a relatedness between these isolates and blood isolates collected at the same hospital, suggesting the presence of a successful hospital strain capable of colonizing inpatients.

Bacterial Epidemiology and Antimicrobial Resistance Profiles in Children Reported by the ISPED Program in China, 2016 to 2020

Pan Fu et al.

Summary: The ISPED program was established in 2015 to monitor and analyze bacterial epidemiology and antimicrobial resistance in children. From 2016 to 2020, a total of 288,377 isolates were collected from 11 tertiary care children's hospitals in China, showing high rates of multidrug-resistant organisms. The study highlighted the importance of effective monitoring and antimicrobial stewardship in children to address the urgent threat posed by MDROs.

Summary: This study isolated 16 carbapenemase-producing Klebsiella pneumoniae strains from a tertiary-care hospital, showing resistance to multiple antibiotics with ST11 being the most common sequence type. Systematic screening tests are needed to identify carbapenemase producers and prevent healthcare-associated transmission.

The changing epidemiology of carbapenemase-producing Klebsiella pneumoniae in Italy: toward polyclonal evolution with emergence of high-risk lineages

Vincenzo Di Pilato et al.

Summary: A nationwide survey in Italy in 2016 revealed that while CG258 remained the most common CR-KP lineage, new high-risk lineages CG307, ST101, and ST395 have emerged. KPC remained the most common carbapenemase among the isolates. This study indicates a polyclonal population of CR-KP in Italy.
