Intranasal drug delivery: opportunities and toxicologic challenges during drug development

Lea-Adriana Keller et al.

Summary: In the last decade, there has been a growing interest in intranasal drug delivery in pharmaceutical R&D. This review article highlights the advantages of nasal delivery for local and systemic drug delivery, as well as for CNS indications, compared to conventional systemic approaches. However, formulation limitations and toxicological considerations remain areas that need further optimization in this field.

Mir-142-3p Regulates Inflammatory Response by Contributing to Increased TNF-α in Chronic Rhinosinusitis With Nasal Polyposis

Xiang Qing et al.

Summary: This study showed that miR-142-3p and TNF-alpha expression levels were significantly elevated in chronic rhinosinusitis with nasal polyps patients, and increased in response to LPS stimulation. Knockdown of miR-142-3p in HNEpCs downregulated TNF-alpha expression at both mRNA and protein levels. It is suggested that miR-142-3p may participate in regulating the body's inflammatory response via the LPS-TLR-TNF-alpha signaling pathway in chronic rhinosinusitis with nasal polyposis.

Inflammatory features and predictors for postsurgical outcomes in patients with nasal polyps stratified by local and systemic eosinophilia

Li Pan et al.

Summary: This study aimed to identify inflammatory patterns and predictors associated with clinical outcomes in chronic rhinosinusitis with nasal polyps (CRSwNP) patients with different blood and tissue eosinophilia. Different inflammatory patterns were observed in CRSwNP patients with distinct blood and tissue eosinophilia, and asthma, prior sinus surgery, and blood eosinophilia were identified as the top predictors for postsurgical uncontrolled disease.

Spotlight on microRNAs in allergy and asthma

Julie Weidner et al.

Summary: In the past decade, miRNAs have garnered scientific interest due to their crucial role in allergic diseases, acting as master post-transcriptional regulators of cellular processes. Dysregulated miRNA expression may contribute to disease development. Research focuses on understanding the specific functions and molecular mechanisms of miRNAs in allergic diseases, particularly in regulating immune responses and responses to environmental factors.

Distinct expression of SARS-CoV-2 receptor ACE2 correlates with endotypes of chronic rhinosinusitis with nasal polyps

Ming Wang et al.

Summary: The study found that ACE2 expression in nasal tissues of nonECRSwNP patients was significantly increased, positively correlated with type 1 inflammation, and negatively correlated with tissue infiltrated eosinophils, IL5, and IL13 expression. IFN-gamma enhances ACE2 expression, while glucocorticoids attenuate this increase.

Integrated miRNA and mRNA expression profiling reveals dysregulated miRNA-mRNA regulatory networks in eosinophilic and non-eosinophilic chronic rhinosinusitis with nasal polyps

Xiangting Bu et al.

Summary: This study identified dysregulated miRNA-mRNA networks in eosinophilic and non-eosinophilic CRSwNP subtypes, suggesting potential therapeutic targets for future management of CRSwNP. The role of specific miRNAs such as miR-154, miR-221, miR-223, let-7, and miR-34/449 families were highlighted in the pathogenesis of CRSwNP subtypes. Real-time PCR validation showed consistent results with RNA sequencing data.

MicroRNA-21 inhibition attenuates airway inflammation and remodelling by modulating the transforming growth factor β-Smad7 pathway

Jung Hur et al.

Summary: Inhibition of microRNA-21 has both anti-inflammatory and anti-remodelling effects in ovalbumin-induced chronic asthma, potentially through the microRNA-21-transforming growth factor beta 1-Smad7 axis. This suggests that microRNA-21 inhibitors could be a novel therapeutic target for allergic asthma, especially in cases with airway remodelling.

miR-21: a non-specific biomarker of all maladies

Ana E. Jenike et al.

Summary: miRNA-21 is considered as a specific predictive or prognostic biomarker for at least 29 diseases, but lacks specificity for any one disease, thus making it not an ideal candidate for a biomarker despite continued evaluation. This issue of non-specificity in biomarker designation is shared with other common, ubiquitous miRNAs and warrants concern for their use as biomarkers as well.

Burden of Disease in Chronic Rhinosinusitis with Nasal Polyps

Claus Bachert et al.

Summary: CRSwNP is a disease with type 2 inflammation that greatly impacts patients' quality of life with severe symptoms. Existing treatments may lead to recurrence of nasal polyps and adverse effects, particularly in patients with comorbid conditions like asthma and nonsteroidal anti-inflammatory drug-exacerbated respiratory disease. New therapies targeting type 2 inflammatory disease pathophysiology are needed to effectively control symptoms and minimize recurrence.

miR-21 and miR-146a: The microRNAs of inflammaging and age-related diseases

Summary: Inflammaging is a key concept in understanding how aging contributes to the development of age-related diseases, with a focus on the role of miRNAs in regulating inflammatory pathways. The assessment of individual genetic make-up alone is insufficient to depict the trajectories of age-related inflammation, and studies have shown that circulating inflammamiRs could be used to measure the degree of inflammaging.
