We present a case report of Neisseria meningitidis symptomatic proctitis in an HIV-negative man who has sex with men attending a genitourinary clinic in Malta. The proctitis was caused by a rare penicillin-resistant unencapsulated N. meningitidis strain of the ST-53 clonal complex.
