Summary: Ischemic vascular disease is a major global cause of mortality and morbidity, and the therapeutic goal of regenerating blood vessels in perfusion-deficient tissues through delivering endothelial stem/progenitor cells has strong scientific basis. However, challenges and contradictions in clinical trials have highlighted the difficulties in developing endothelial stem cell therapies. Emerging evidence raises questions about the traditional view of bone marrow as the source of these cells and suggests the vascular wall as the niche for tissue-resident endothelial stem cells.

Summary: Extracellular vesicles (EVs) secreted by nucleated cells, especially endothelial progenitor cell-derived EVs (EPC-EVs), have shown beneficial effects in various diseases such as cardiovascular and lung disorders, accumulating at injury sites with angiogenic and restorative effects surpassing their ancestors. This review article focuses on the biogenesis, delivery routes, and long-term storage methods of EPC-EVs, highlighting their anti-inflammatory, angiogenic, and tissue protection effects in diseases, and discussing the therapeutic potential of EPC-EVs in clinical applications.

Extracellular Vesicles Derived From Regeneration Associated Cells Preserve Heart Function After Ischemia-Induced Injury

Summary: This study demonstrated that under vasculogenic conditioning, peripheral blood mononuclear cells shifted their phenotype to regeneration-associated cells, which showed superior therapeutic efficacy compared to mesenchymal stem cell-derived EVs in the context of myocardial ischemia reperfusion injury. RACev significantly enhanced cell viability and proliferation, improved cardiac functions, and reduced fibrosis. The beneficial effects were associated with the delivery of crucial miRs related to angiogenesis, anti-fibrosis, anti-inflammation, and cardiomyogenesis by RACev.

Summary: The study investigated the effects of EPC-Exos-derived microRNAs on myocardial infarction and found that miR-363-3p and miR-218-5p enriched in EPC-Exos can ameliorate cardiac fibroblasts proliferation and angiogenesis by regulating p53 and JMY. Treatment with exosomes containing miR-218-5p or miR-363-3p mimic improved left coronary artery ligation-induced MI in rats by targeting the p53/JMY signaling pathway.
