Mapping Actuarial Criteria for Parkinson's Disease-Mild Cognitive Impairment onto Data-Driven Cognitive Phenotypes

Summary: Prevalence rates for mild cognitive impairment in Parkinson's disease (PD-MCI) remain variable, and inconsistent operationalization of normative cutoffs for cognitive impairment is a primary factor. This study aimed to determine the optimal cutoff for classifying individuals as PD-MCI by comparing classifications against data-driven PD cognitive phenotypes. The 1.5 SD cutoff produced the best model of cluster membership and resulted in the best alignment between PD-MCI classification and the empirical cognitive profile containing impairments associated with greater dementia risk.

Cognitive Decline in Parkinson's Disease: A Subgroup of Extreme Decliners Revealed by a Data-Driven Analysis of Longitudinal Progression

Summary: In this study, researchers aimed to identify latent sub-groups of longitudinal trajectories of cognitive change in Parkinson's disease patients. Results showed that trajectories of cognitive change were best described by two classes: a large subgroup with stable cognitive performance and a small subgroup with rapid decline. The rapid decliners had lower CSF amyloidss42 levels, a higher prevalence of sleep disorder, and pronounced loss of caudate DAT density at baseline, suggesting the existence of a distinct minority subtype of Parkinson's disease with rapid cognitive decline.
