Clinical Importance of Left Atrial Infiltration in Cardiac Transthyretin Amyloidosis

Summary: This study characterized left atrial pathology in explanted hearts with transthyretin amyloid cardiomyopathy (ATTR-CM); assessed left atrial mechanics using echocardiographic speckle tracking in a large cohort of ATTR-CM patients; and studied the association with mortality. The findings revealed extensive atrial infiltration of ATTR amyloid, loss of atrial function, and increased stiffness, which were strong independent predictors of mortality. A distinctive phenotype named atrial electromechanical dissociation (AEMD) was identified in patients in sinus rhythm with poor prognosis.

Multi-chamber speckle tracking imaging and diagnostic value of left atrial strain in cardiac amyloidosis

Summary: Cardiac amyloidosis (CA) affects all four heart chambers, and strain parameters of all chambers are altered in CA patients, particularly those with amyloid transthyretin (ATTR; n = 144, 34%) or amyloid light-chain (AL; n = 117, 28%) CA. Peak left atrial longitudinal strain (LA-PALS) is independently associated with the diagnosis of CA or ATTR-CA beyond standard echocardiographic variables and cardiac biomarkers. Patients with the most severe impairment of LA strain are most likely to have CA or ATTR-CA.

Morpho-functional changes of cardiac telocytes in isolated atrial amyloidosis in patients with atrial fibrillation

Summary: Telocytes in patients with atrial fibrillation and atrial fibrillation with isolated atrial amyloidosis exhibit ultrastructural signs of synthetic, proliferative, and phagocytic activity, and show various immunophenotypes. Despite no significant differences in morphology and immunophenotype of telocytes among patients with different forms of atrial fibrillation, CD68-positive cells are more commonly detected in patients with atrial fibrillation and isolated atrial amyloidosis. In aged patients with atrial fibrillation and isolated atrial amyloidosis, remodeling of atrial myocardium may lead to the transformation of telocytes into transitional forms with characteristics of fibroblast-, histiocyte-, and endotheliocyte-like cells.

With advances in imaging techniques, non-invasive diagnosis is now possible, showing that cardiac amyloidosis is more common than previously thought. The Working Group on Myocardial and Pericardial Disease has proposed definitions and diagnostic algorithms for cardiac amyloidosis.

Among the strain values, peak longitudinal LA strain showed the strongest association with prognosis.
